If I'm not pregnant, what's going on?

	Asked by jltj10 (Female; 32; No recorded obs, high prolactin 3 years ago, this now normal, low progesterone 6 months ago, current level unknown. Tested for hypoglycaemia due to dizziness 6 months ago, negative result. Thyroid function, liver function, blood sugar, cortisol, other hormones all monitored and normal, last tested July. 	I am not registered with a local doctor currently which is why I'm using this service. I'll try to keep this brief. My husband and I have been trying for a baby for around 3 years. I am 32, him 48. I usually have very consistent periods, lasting 7 days and pretty heavy. Some breast tenderness leading up to the period disappears as soon as I start bleeding. This time (2nd Nov) I came on quite lightly in comparison, and it lasted 3 days before stopping suddenly. My breasts increased in size and tenderness over the next few days until I couldn't bear to be hugged. I was also very teary. 4 days ago I began feeling sick, headachy and had diarrhea a few times. I felt incredibly ill and nauseous at night and early morning and couldn't bear the taste or smell of anything. I threw up a few times. My saliva increased which made things worse. Between 12pm and 8pm I was absolutely fine and could eat. I managed to sleep last night but now I'm quite shaky and dizzy. I still have fuller and sorer breasts than usual although this has abated, my saliva is still up and I feel a tiny bit sick, but I'm eating OK. This has never happened before, I have never felt like this, it's like my hormones have spiked. I have taken 4 home pregancy tests and all read negative. Please help.

While I was reading your post, a few possibilities came to my mind which I will try to explain one by one. See since you have been trying to conceive since three years and have such soreness of breasts, this raises the possibility of decrease of progesterone levels. You have already had low progesterone levels earlier which have not been evaluated recently. Any further deterioration in such levels will cause symptoms like infertility, fibrocystic breasts, irregular periods and breast tenderness. Soreness of breasts can be attributed to fibrocystic breast changes which can cause fullness, severe breast pain or mastalgia. Secondly, the GI (gastrointestinal) symptoms can be independent of these breast changes (which can be due to low progesterone levels) and can be due to gastroenteritis. Thirdly, since you have been trying for pregnancy and your last menstrual period being 2nd November, so these can be the earliest changes after conception. Your urine for pregnancy test will not come positive at this stage. Most chemical tests for pregnancy look for the presence of the beta subunit of hCG or human chorionic gonadotropin in the urine. hCG can be detected in urine after implantation of zygote in the wall of the uterus formed by the union of the sperm & the ovum, which occurs six to twelve days after fertilization. Urine for pregnancy test is a qualitative test and will not show positive if the levels of HCG are low which usually happens in the first few weeks of pregnancy. So a quantitative test like serum HCG levels or highly sensitive urine for pregnancy test like aimstrip test should be done at this stage which can detect even low amounts of HCG. Increased saliva or drooling is common in vomiting as well as hyperemesis gravidraum. So the approach should be like this: 1) Consult a gynecologist and get serum HCG levels and progesterone levels done. 2) Apply warm compresses on the breasts and keep yourself well hydrated. 3) Drink plenty of fluids, juices and soups. We cannot prescribe any medicine at this stage because of the uncertainty of the status of pregnancy which has to be cleared first. So a gynecologist’s consultation is crucial at this stage. There is a possibility that you conceived in this cycle. However, generally the symptoms of pregnancy appear after the implantation of the zygote in the uterus. The pregnancy test will be positive after the missed period only. So take one on the next due date. The other possibility is that you have a bad case of stomach flu. You can take antacids and see. This will not harm the baby if you take them. Eat light and drink fluids. Another possibility is severe hormone imbalance. You need a complete assessment of this. Also if you have been trying for 3 years, it would b e best to register under an infertility specialist and get everything investigated. It is difficult to comment beyond this at this stage.
