Future job gave me a sheet for my doctor to fill out. Why x-ray, urinalysis, wasserman test, special test?

My future employer is asking me to get a physical. They gave me two pages for the doctor to fill out. The first page is question about previous questions. The second page is for him/her to fill out. It has things like pulse blood pressure, etc. Then it has a space and has headings and lines the headings are glandular system, urinalysis, temperature, hemoglobin, wasserman test, x-ray, and special tests. Will all of these things be done? What are they looking for in urinalysis and x-ray?

Hello luke123456, The investigations are important for insurance and detection of any incidental findings,early detection of hypertension,diabetes,cardiovascular diseases. These are done as preventive measures and most of the companies go for a previous medical health examination test to rule out any complications and communicable diseases. Best
