Answer by Dr Bhupinder K, MD (doctor) on Mon 18, Jan 2010 11:00am:
Hi,
Welcome to the AMD forum. Many thanks for posting on it. Members of the “streptococcus milleri” group are a
part of the normal flora of the mucous membranes and uncommonly can produce significant infections. They are
commonly found in the pus forming infections and majority of such infections are polymicrobial (multiple
bacteria) in nature. It is referred to as streptococcus inetermedius group which contains three distinct
streptococcal species including Streptococcus intermedius, Streptococcus constellatus, and Streptococcus
anginosus. Streptoccoccus milleri is a viridians streptococcus which is usually beta hemolytic and produces
pyogenic infections. Most strains have been found to be susceptible towards penicillins, ampicillin,
erythromycin, and tetracycline. So these antibiotics are one of the best antibiotics for this bacterium. Some
other antibiotics which have been found effective in vitro are penicillinase resistant penicillins,
cephalothin, cefamandole, rifampin, vancomycin, clindamycin and chloramphenicol. Penicillin belongs to beta
lactam group of antibiotics and has been found to be very effective. I hope it helps. Take care and please do
keep me posted in case you have any additional doubts. Kind regards. I am forwarding you a very good review
about this bacteria which has been published in the American society of microbiology. You can find detailed
information from this site: http://cmr.asm.org/cgi/reprint/1/1/102.pdf

Hi Mattaremw!
Dr Bhupinder has rightly guided you regarding the antibiotics that are effective against “streptococcus
milleri”. She has also provided a very good review for this. I just want to add one point. For any organism,
the best way to know which antibiotic will be effective, is to stop all antibiotics (if you are taking any)
for at least 72 hours and then set up a sensitivity test. Usually a culture sensitivity test is done. If the
organism has been identified, then culture s not required. This sensitivity test identifies the effect of a
number of antibiotics against the organism. The antibiotics having the maximum effect to those not having any
effect are recorded. Thus you can know which antibiotic will be effective the most. Hope this helps. Take
care!
