Asked by Unregistered (Female; 45; USA; Hypertension; Relevant drugs:Altace, 5mg (1X), Labetalol 200mg (3x), Norvasc 5mg (1x), Hydralazine (3x) ) on Sun 17, Aug 2008 12:14pm

I'm a 45 year old woman who has stubborn hypertension. I was first diagnosed when I had my second child when I was 33. My pressure went up during delivery (C-section). Since then my blood pressure has been unstable. It's been a year since I've been on medication. I'm currently taking Altace 5 mg. (1x), Labetalol 200mg.(3x), aspirin 80mg (1x), Cartia (1x), however, on my last dr. visit a few days ago, my doctor has changed the Cartia to Norvasc 5mg (1x) and has added Hydralazine (3x). I haven't yet started the Norvasc and Hydralazine, so I'm not sure what effect I'll have on that. My pressure has been normal with the last medications I was taking, however, when I went to the dr.'s office, it went up to 180/90. The dr. told me that apparently I have "white coat syndrome" and prescribed the extra medication. All of my blood test came out negative and I have not apparent conditions. I am looking for a hypertension specialist in my area and haven't had much success. Could you please send me a list of doctors that I may go see regarding my hypertension. Thankyou.

Answer by Dr Smitha M, MD (doctor) on Tue 02, Sep 2008 09:44pm:

Hi, White coat syndrome is one where a patient normally has moderate or normal blood pressure but has an increase in blood pressure on seeing a doctor (white coat) or is examined in the hospital setting. This indicates that the rise in BP recorded is due to anxiety. It would be ideal if you can measure your blood pressure at a regular time at home or near your home where you would not have the added stress of going to the hospital or seeing a doctor. If you had preeclampsia or eclampsia, it would be good to have your renal perfusion studied with doppler study, but this should be done on the advice of your treating doctor. This is because there has been increasing evidence that insults to the kidney can be key to the development of hypertension and resistant hypertension. Do keep us posted on any further doubts in this regard.
