Answer by Dr Heinrik M, MD (doctor) on Fri 14, Mar 2008 04:58pm:

Hi, The liver involvement is deemed secondary disease, with the stomach cancer being the primary. The treatment would be based on the behavior of the stomach cancer. However, if there is impending complications from the liver, therapy directed primarily for the liver would be contemplated (this may include inserting tubes to relieve obstruction). The use of the term “secondary stage” may be inappropriate because it may cause confusion with disease staging. Stomach cancer with liver involvement is not stage 2 disease but stage 4 disease. The prognosis is different between a stage 2 and a stage 4. The treatment would also be different. It may be that the initial goal of chemotherapy was to reduce the cancer to have surgery subsequently. With liver involvement, the goal of the chemo may be to improve symptoms and survival.
