There are two mechanisms on how excess weight or fat affects the menstrual cycle. The first is thought to be
that increased fat would translate to increased estrogen in the body that causes endometrial proliferation.
If the proliferative effects is timed while the menses are ongoing, this could mean continues shedding of your
endometrium, thereby prolonging the menstruation duration.
Another mechanism, if increased estrogen comes after menstruation, is that ovulation may be suppressed, and
therefore delay or prevent the onset of menses.
I would suggest additional evaluation by your gynecologist including transvaginal ultrasound to assess for the
endometrial thickness, and you may also discuss with her regarding actual measurement of estrogen levels. The
ultrasound may also demonstrate the presence of an abnormal occurrence called endometriosis which can present
with prolonged menses.
